Introduction Modified Brostrom-Gould surgery (MBG) aims to repair the lateral ligaments of ankle in patients with ligamentous laxity and chronic instability. surgery-the Brostrom technique associated Gould augmentation-is currently gold standard surgical option for instability worldwide. Chronic caused by sprains is one most common sports-related injuries, commonly recommended as operative treatment. While arthroscopic becoming more heavily favored approach choice, open still pertinent whom unsuitable. Aim This paper discusses a modified mini-open smaller incision (1.5 cm) study outcomes this on patients' post-operative pain, stability, functional outcome. Methods Forty-two were followed up mean 2.6 years after undergoing surgery. The Visual Analog Scale (VAS), Foot Ankle Outcome Score (FAOS), Karlsson scores used monitor their recovery. Wilcoxon signed-rank test SPSS Statistics (v.28.0.1) software data management analytics. Results results showed score 83.4, FAOS 69.7, VAS 1.33. These are comparable studies conducted conventional repair. Conclusion provides favorable outcome reduction pain suggests no decrease efficacy approach. coupled added advantages wound, better wound healing outcomes, availability not suited
